Cranioplasty in pediatrics often results a poor outcome. Therefore, proper consideration and decision-making are needed choosing surgical techniques for cranioplasty children, especially autologous, which can cause abnormal skull brain growth. A 3-year-old girl presented to our outpatient clinic with complaint of left front-temporo-parietal defect due history acute subdural hemorrhage evacuation decompressive craniectomy. After series failed cranioplasties, the patient then underwent split calvarial cranioplasty. Postoperatively, was fully conscious had no neurological deficit. Commonly used autologous material bone graft is those rib calvarial. Large area does not allow use rib. calvaria main choice this case, its capability giving larger obtained, more suitable contour area, single incision, rigidity cosmetic purpose. Autologous be certain cases infection or trauma that causes tissue damage, so autograft flap cannot reused. In pediatric cases, technique appropriate children has problem defect.
